Faircape Health is extending our subacute care and rehabilitation services to one of our long-term healthcare facilities in Somerset West, Heritage Manor. We are catering to patients in need of short-term, long-term, dementia, respite, and palliative care within a secure and nurturing environment.
Our world-class care aims to bridge the gap between hospital and home. Our Multidisciplinary Team (MDT) of leading healthcare professionals administers care in the most therapeutic environment possible and supports healing outcomes in all aspects.
We seek a dedicated (Locum) Doctor to join and assist our team over the weekend by, prioritizing patients' medical stability and enhancing their rehabilitation progress. This role entails managing rehabilitation patients and addressing long-term geriatric needs, particularly focusing on rehabilitation from various conditions such as post-surgery recovery, acute hospitalisation, stroke, total hip replacement, and dementia.
Collaborate closely with our multidisciplinary team of experts in physiotherapy, occupational therapy, speech therapy, social work, dietetics and nursing staff to ensure a seamless transition to rehabilitation and safe discharge.

Duties include but are not limited to:
- Upon patient admission, conduct comprehensive assessments of patients' medical conditions, including physical examinations, review of medical history, and diagnostic tests to develop appropriate care plans.
- Develop and implement treatment plans tailored to each patient's specific needs, including medication management, therapy, and other interventions aimed at improving health outcomes.
- Collaborate with a multidisciplinary team of healthcare professionals, including nurses, therapists, social workers, and specialists, to coordinate patient care and ensure continuity of treatment.
- Monitor patients' progress closely, evaluate the effectiveness of treatment plans, and make adjustments as necessary to optimise outcomes.
- Provide patients and their families with information about their medical conditions, treatment options, and self-care techniques to promote health and prevent complications.
- Maintain accurate and up-to-date medical records, including progress notes, treatment plans, and discharge summaries on Healthware (electronic platform)
- Participate in quality improvement initiatives aimed at enhancing the delivery of sub-acute care services, such as developing protocols, conducting research, and implementing best practices.
- Provide urgent / emergency telephonic cover to the nursing staff in medical emergencies as needed, providing timely and appropriate medical interventions to stabilise patients' conditions and prevent further deterioration. No routine call outs from home are required.

Requirements:
Qualifications:
- Bachelor of Medicine & Bachelor of Surgery (MBChB)
Experience and Knowledge
- Proven post-community service
- Proven experience in internal medicine, family medicine, geriatrics, palliative care is beneficial.
- Experience working within a multidisciplinary and nursing team environment advantageous

- Crisis Management Abilities, the ability to remain calm under pressure and effectively coordinate response efforts to ensure the safety and well-being of patients and staff.
- Strong interpersonal skills, ability to build and maintain positive relationships

- MPS / insurance membership and active HPCSA registration.
Benefits:
Weekends: 08h00 until patient rounds are completed (6 weekend days a month for approximately 5 hours).
We require our Doctors to provide urgent / emergency telephonic cover when the need arises to the nursing staff. Our nursing staff are very competent in managing most issues and only call for emergencies on rare occasions. No routine call outs from home are required.
Weekdays: An hourly rate of R536.00
Weekends: A daily on-call rate of R1183.00 for Saturday and R1774.00 for Sunday and an additional fee of R178.00 per patient
