Job Summary:

To provide clinical pharmaceutical services at hospital facilities in accordance with policies, standards and regulations

Job Requirements:

Lead assigned patient service lines and therapeutic programmes

In collaboration with Group Clinical Pharmacist, ensure the adoption of medication utilization frameworks, standards and system at hospitals.

Facilitates pharmaceutical care services, direct patent care programme, medication utilization systems and care areas to assure medicine utilization activities are aligned with patient care needs, best practice and regulatory standards.

Monitor and review profiles / chart to identify, prevent or mitigate drug related problems, improper medicine use or dose selection, over-indications, medication without indication and treatment failures.

Implement stewardship activities and surveillance programmes.

Track and evaluate assigned pharmacy programmes at the facility for quality and routinely benchmark against local and international best practice.

Ensure clinical pharmacy is an integral part of health-care delivery system by actively collaborating with healthcare providers within the facility.

Work within established parameters, protocols and frameworks for medicine use and control.

Identify high risk patients and implement interventions to improve quality and safety.

Make appropriate evidence based, patient-centered medicine recommendation.

Provide pharmaceutical services throughout the hospital facility

Ensure all transactions are recorded and reported on the approved platforms and systems.

Provides accurate, safe and timely medicine therapy based on individual patient need.

Follow the established escalation processes pertaining to clinical pharmacy.

Ensure recommendations consider cost beneficial initiatives adopted in the group
